New Hire Paperwork

Welcome aboard!  We look forward to having you join the GHS Team.  In order to prepare for your arrival, we need you to complete the new hire payroll paperwork prior to your attending New Employee Orientation.  Please print each "required" document, complete in full and submit to the Employee Health Nurse when you arrive for your post offer health assessment.  Failure to complete the forms prior to your start date will delay your being added to GHS payroll system.  Please contact us if you have any questions, issues, or concerns.

Please do not forget to bring a voided check, savings deposit slip or letter from your bank (on letterhead) which includes your routing and account numbers.

Note - if you have not completed the Authorization of Release Form so that a background check can be started, please download, complete and fax the form to the HR Recruiter that is processing you for hire.  Only submit this form if have been made a job offer and asked to do so!

Proof of Residency

In accordance with SC Law we must complete a "Proof of Residency" verification if you are assigned to work at a GHS long-term care facility, Home Health department or Subacute Unit; both a state background check and a federal criminal background check (fingerprinting) shall be required if the applicant is unable to verify S.C. residency or residency in another state for twelve (12) consecutive months prior to the employment application date.  Once the state criminal background has been completed and the fingerprinting initiated, the applicant is eligible for employment, pending the results of the federal criminal records check.

  • To determine if an applicant must have both types of background checks, he/she must first provide GHS with proof of residency to verify that he/she has been either a resident in S.C. for the past 12 months or in his/her resident state for the past twelve months.  The 12 month period is determined from date of the employment application.  The applicant can provide verification of residency through:
  1. A driver’s license or identification card issued by the state;
  2. Rent, mortgage or utility receipts in the applicant’s name for a home within SC or his/her resident state.
  3. Pay stubs in the applicant’s name from a business located in S.C. or his/her resident state.
  4. Bank records in the applicant’s name showing a deposit or checking account held in S.C. branch office of a bank or in his/her resident state;
  5. Income tax return: or
  6. Voter’s registration card.

If the applicant can prove S.C. residency or his/her resident state, then only a state criminal background check is required.”

SLED Criminal Background Checks

A SC criminal background check verification is required for all new hires and employees transferring to work at Cross Creek Survery Center, Patewood Surgery Center and GMH Outpatient Surgery locations.
